| Making
an Appointment
Our office hours are Monday through Friday 8:30 A.M. to 5:00 P.M.
at all locations. You may call during these times to schedule an
appointment. We see all patients on an appointment basis. If you
are unable to keep your scheduled appointment, please give a 24-hour
cancellation notice.
When scheduling appointments,
please be aware that during you initial visit and occasionally thereafter,
your eyes may be dilated. Please bring sunglasses. Although many
people can drive after dilation, we encourage you to bring a driver
for your return home after the appointment.
Fees,
Billing, and Collection
All non-covered services, deductibles, and co-payments are due at
the time of service. We will be happy to discuss fees in advance
and provide an estimate on any service we provide. We accept cash,
check, MasterCard, and Visa.
Filing
Insurance
We participate in many insurance plans, including Medicare, Medicaid,
and most HMO's and PPO's in the area. We will file with the insurer
for you. However, you remain personally responsible for any portion
of the bill not covered by insurance. It is your obligation to thoroughly
understand the coverage and exceptions of your particular policy.
Patients having HMO and PPO plans that require a referral number
must contact their Primary Care Physician (PCP) for that number
prior to your visit. The insurance companies will not allow our
office to contact your PCP for you. You will be responsible for
all charges if you fail to obtain a referral number.
Laser Vision Correction
is rarely covered by insurance. We recommend that you contact your
insurance company prior to your visit to find out if Laser Vision
Correction is a covered benefit. Financing is available to qualified
candidates. Our Laser Vision Coordinator can help you find out about
this option.
Prescription
Refills
All prescriptions and authorization for refills should be requested
during normal office hours.
Emergencies
Our practice can be reached 24 hours a day. After hours, your call
will be received by our answering service, which will contact the
eye doctor on call for our practice. If you have any difficulty
reaching the on-call physician and you feel you have a sight-threatening
emergency, please go to the nearest hospital emergency room.
Contact
Lenses
Our practice fits and prescribes all types of contact lenses, including
difficult and hard to fit cases. Our prices are competitive with
all major retailers in the area. Please note that there are extra
professional fees for a contact lens exam, in addition to the regular
exam fees for glasses. These extra fees are necessary because a
contact lens exam requires more professional time, extra tests are
performed, and in some cases a follow-up visit is required. Most
insurance companies will not cover the contact lens portion of the
exam. You will be responsible for paying this portion of the exam
at check-out. Please contact our office regarding the current fees.
